身体检查证明：请下载《外国人身体检查表》，去当地正规医院完成检查并请医师用中文或英文完成该表格，同时加盖医院的公章（公章需部分盖印在本人照片上），有效期在体检之日起六个月内。
Physical examination record: please download the Foreigners Physical Examination Form, go to a local hospital to complete the examination and ask the physician to complete the form in Chinese or English, and stamp the official seal of the hospital (the official seal must be partially stamped on the ID photo). The date of the examination must be within six months from the date of uploading.
